Portal Agreement
Maxim at Home, LLC d/b/a VilliTM ("Villi") has established a secure, user-friendly platform, in which our clients and their representatives are able to:
- Request self-directed, private-pay companion services
- Search and select personnel for assignment
- Select the services needed
- Review their Calendar of Services
- Remit payment for services
The Portal may not be used to report or inquire about service issues or emergent issues/concerns. These communications must be reported, by phone, to VilliTM.
Please note you are responsible for informing VilliTM of any change in or issues with your email account which may impact your communications with VilliTM.
Privacy/Security. The Portal is managed by VilliTM and is compliant with state and federal privacy/security regulations.
While all efforts have been made to create and maintain a secure environment, on-line communications and activities carry an element of risk.
For example, someone may impersonate VilliTM by sending emails in our name or the names of our employees in order to obtain your access information and/or access to your records.
By using or accessing the Portal, you expressly accept these risks and agree that VilliTM is not responsible for breaches of confidentiality caused by you or an independent third-party.
Please read our Notice of Privacy Practices at https://www.myvilli.com/privacy-policy/ for additional information on your rights to access and restrict access to your personal information as well as VilliTM practices on how client information is used and safeguarded.
You are responsible for taking the necessary steps to protect yourself from on-line intruders.
- Create a unique/complex user name and password for access to your account
- Secure your access information and do not share it with any other persons.
- Do not access the Portal from a public computer or from areas outside the United States.
- Log out and close your browser when you are finished.
If you suspect you have received a fraudulent solicitation/email from VilliTM, do not open the message or click on any links.
Contact the VilliTM privacy/security team in order to verify the message and content.
If you find out after-the fact you have been phished, update your access information immediately and inform contacts in your system that may have been impacted.
How to Report Phishing. If you got a phishing email or text message, report it.
Miscellaneous. This Portal is provided as a courtesy to our clients and their legal representatives at no cost and is only available in English at this time.
The Portal may be modified, revised or upgraded from time to time by VilliTM without notice or liability.
VilliTM may change, suspend, or discontinue any aspect of the Portal at any time, including, but not limited to Portal content and access.
User Acknowledgement and Agreement. I acknowledge that I have read and fully understand this User Agreement.
I have been given risks and benefits of the Portal and agree that I understand the risks associated with online communications between VilliTM staff and myself, and consent to the conditions outlined herein.
I agree to adhere to the policies set forth herein, as well as any other instructions or guidelines VilliTM may impose for online communications.
I have been given an opportunity to ask questions related to this Agreement and all of my questions have been answered to my satisfaction.
